In-Reach Support

Supporting individuals within prison settings as they prepare for release and reintegration.

Preparation before release can play a critical role in successful reintegration.

CAP’s In-Reach support works directly with individuals in custody to help build confidence, plan ahead, and connect with supports before returning to the community.

About The Service

In-Reach support focuses on early engagement and preparation for release.

Our team works with individuals to identify needs, discuss goals, and support planning around housing, services, wellbeing, education, employment, and community reintegration.

This approach helps create continuity between prison-based support and community-based support after release.

Support may include

Pre-release planning

Goal setting and support planning

Preparing for transition and reintegration

Why Early Support Matters

The transition from prison to community can feel uncertain and overwhelming.

Providing support before release can help individuals feel more prepared, informed, and connected to appropriate supports and opportunities.

Supporting Reintegration from the Start

Our In-Reach team works to ensure individuals are not left to navigate release and reintegration alone.
